Camper Forms

• If you need hard copies of the following forms mailed to your home address, please contact the camp office by phone or email.

• Please do not mail or email to camp. Bring all documents on the day of registration. 

• Make copies if needed, the forms will not be returned at the end of camp. 

• New forms are required every year. We cannot search for old medical records to transfer from year to year.
