FORMS: 
Please print the forms below and bring them completed to Camp when you check in on your first day of Camp.

• Medical Form (Link includes Form A, Form B & Form C)  
  • Health Form A:  Health History and Treatment Authorization:  Must be completed annually by a parent/guardian of a minor camper or by an adult camper/staff member. 
  • Health Form B:  Must be completed every 3 years by a licensed medical personnel.  
  • Health Form C:  Authorization for the Administration of Medication.
